Looks like a late '60s Dodge CrewCab with an M715 bed on M211 axles, possibly the whole chassis with that odd skinny bumper in front. Could also be the M715 W/W LU-4 and bumper setup slapped on.

Those are 46" tall Goodyear MVT tires too. The axles look big as well.
