I'm on my forth call out of the day because of the cold weather so have not spent any quality time looking for old International transmissions in my collection.

So for anyone following along, I did get up to the truck today and verify with the shifter off I can manually shift the trans into reverse and first.
So this tells me that either there are two different caps for the trans, or I have the wrong shifter.

Just as an update on this, a member over on binder planet sold me a different style cap, and voila the transmission goes into all the gears now.
"New" one is on the left.
I think I probably just had the wrong cap/shifter combo.
