My thought was/is possibly a squib followed by an out of battery shot. Not sure if that sequence is really feasible though. Looks like the brass from the head is almost all gone.

Was the kid shooting a 20 gauge? A 20 gauge shell will drop in a 12 chamber and a 12 can go right behind it. Causing catastrophic damage just like that.

Doubtful. "kid" is a 19 or so yo experienced hunter. I doubt there were 20ga around anywhere. Big kid, so no reason he would be shooting 20ga at any time vs more pellets in 12ga
 
Any chance he was shooting reloads?

I'm sure there is a chance, but I don't know anyone that reloads shotgun shells. They just feel like commercial ammo peeps. He was at their 1500acre high fenced "farm" on the MS River where they have managed for deer and ducks for years

Mud in the barrel. Spit parts out the bottom, side, and slit the barrel.
