Hmmmm was it tight enough? I think a 3/4" Grade 8 bolt needs to be torqued around 275ish lbs/ft.
You could have had a bad bolt, but if you bought it at a reputable place, that's unlikely as well.

Hiems and bolts need to be replaced on a time basis. Monster jam would normally replace them every year or 2, but those are in a higher stress class. Those trucks only run 1.25” hiems for links. My $0.02
