I have a later model (98+) NV4500 23 spline. I'm reading conflicting stories. Can I just swap the output? Or what else comes into play?

You should be able to stick a 23sp into that. A hell of a lot easier and cheaper.
I think he has one that's 23sp and wants to swap it to 29sp..... If I'm reading his post correctly.

While I've never managed to break a 23 spline anything. Is there a potential issue behind a 12V? Odds are I couldnt leave it stock.

You said you have a 23spl nv4500 and a 29spl 241.
You didnt say you where putting a 23spl behind a 12v.
Since you need to swap input any how you should be able to do the output shaft at the same time nbd.

My mistake for not delving all the info. 1996 dodge CTD auto, I have a 23sp nv4500, want to switcheroo.
Is it as simple as swapping output shafts, or what else is involved?
You should be fine just swapping the main shaft. I would buy a rebuild kit and replace the synchros and bearings while you have it apart but thats up to you. I would at the very least buy a rear bearing and race with the new main shaft.

Youre gona need to do the input aswell. 23sp nv4500 was v8 gas only, v8 and diesel input are different also.

Good time to go straight to the 1-3/8” input shaft upgrade.
this, unless youre on a budget and using a clutch you already have, if youre buying a new clutch go with the upgraded input
