That kind of failure is actually what I'm imagining with suspect pockets in the concrete, although I do agree that looks like some sort of overlay was involved. If the pocket of loose material is large enough, I can see it just pulling a chunk out above it like that.

I worked at a shop that had a similar failure caused by trying to lift a box truck full of tires on a 9k asymmetrical lift. Boss was warned, boss was a dumbass and told us to lift it.
Just wanted to pop in and say thanks for the replies - my level of concern went up quite a bit as a result of posting this thread. So I'm glad I did
I'll post the solution here whenever that comes about - hopefully not too long from now. Seems like the best bet would be getting a concrete company other than who poured the slab to come out, and go over all the details with them on how to get this dealt with.
